This online needs assessment quiz was designed to help you determine if you or a loved one could benefit from memory care services. Memory care is specialized care provided to those with varying degrees of dementia, Alzheimer’s Disease, or cognitive impairment.
The simple test includes questions about instances of forgetfulness, difficulty with comprehension, and the ability to perform self-care and household tasks. There are twenty-five questions. Please choose the answer that is most appropriate for your situation. Once submitted, you will receive a numeric score that you can use to help determine if memory care services may be right for you.
