General FAQ

Why should I choose a Life Plan Community?

Life Plan Communities, unlike stand-alone independent living communities, offer a continuum of care services. These may include Assisted Living, Skilled Nursing and Rehabilitation, and Memory Care. This range of living and care options is the most flexible long-term choice for both active independent living and future health care.

What are the different levels of support at The Osborn?

Independent Living:
Individuals who can live independently have several options for a maintenance-free lifestyle. Most communities have a minimal age requirement of 65 for eligibility. The Osborn’s independent living includes Garden Homes and apartments, with a 100% refundable entrance fee and monthly fee, and rental apartments with no entrance fee.

Assisted Living:
A team of licensed, caring professionals provides help with daily living activities such as grooming, bathing and medication management. Residents have just the right balance of independence and assistance. This allows them as much control as possible over their lives, while ensuring they get the assistance they want and need.

Memory Care:
Sometimes called memory support or dementia care, memory care is specialized care for individuals with Alzheimer’s Disease or other forms of dementia. Residents live in private rooms, with supportive services and activities. Specially trained professionals customize memory care services to your loved one’s individual needs.

Long-term Skilled Nursing:
When a loved one needs round-the-clock care, skilled nursing is an excellent long-term care option. Staff members include Physicians, Registered Nurses and Certified Nursing Assistants. Services are provided at dedicated skilled nursing facilities or at skilled nursing centers within a Life Plan Community. The Osborn provides round-the-clock skilled nursing in a homelike atmosphere in The Osborn Pasciuti Pavilion, located on The Osborn campus.

Short-Term and Outpatient Rehabilitation:
We offer everything you need to recuperate, gain strength and get back to feeling fine and functioning at your best when you’re at home. This includes speech, physical and occupational therapies to optimize function, especially after orthopedic surgery, stroke or hospitalization for neurological, cardiac, pulmonary or complex medical conditions. Services are available with direct admittance and no entrance fee.

Osborn Home Care:
The exceptional personal care services that we offer Osborn residents can be provided in the homes of Westchester County, New York and Fairfield County, Connecticut residents. Osborn Home Care is also available to residents on The Osborn campus.

Are there tax advantages associated with a move to The Osborn?

Although tax laws are subject to change, many residents of senior living and Life Plan Communities can deduct a portion of their entrance or monthly fees as medical expenses. Rules vary by state and individual circumstances, so we advise you to consult your tax advisor to determine what’s deductible for you and your family.

I have long-term care insurance. Can I use it when I move to The Osborn?

Your long-term care insurance can work hand-in-hand with The Osborn. When you access any of our health-related services, such as Assisted Living, Memory Care, Skilled Nursing, Rehabilitation, or Home Care, you may receive benefits directly as the policyholder.  

Long-term care insurance typically does not cover Independent Living costs. Review your specific policy with your insurer (and our team if needed) to confirm eligibility and benefits before moving to The Osborn. 

What’s included in the entrance fee?

The Osborn’s entrance fee is a one-time, 100% refundable payment at move-in. The amount depends on the size and type of residence you choose, secures priority access to our full continuum of on-campus care (Assisted Living, Memory Care, Skilled Nursing, Home Care), and funds community maintenance and amenities. Many residents use proceeds from selling their home. Your entire entrance fee is refundable to you, your partner, or your estate.

What’s included in the monthly fees?

The Osborn’s set monthly fee covers campus amenities, daily services, community upkeep, 24-hour security, and more. You also skip the costs and daily upkeep of home ownership, such as property taxes, repairs, yardwork, and fluctuating utility bills. With our monthly fee, even lightbulbs and doorknobs are included.

Independent Living FAQ

How does a Life Plan Community differ from a Rental Community?

Life Plan Communities, such as The Osborn, provide their residents with a full continuum of health options in one campus, as well as access to a wide range of services and amenities. At The Osborn, we provide peace of mind in knowing that health services are available, should the need ever arise. 

In contrast, most rental communities do not include health services for their residents. If they do include health services, there is no guarantee of priority access or pricing for residents.  

Since medical services are generally not offered at a rental community, residents requiring medical assistance while they recover from injuries or illnesses are often required to move from their home during their greatest time of need, resulting in a physical adjustment in lifestyle, as well as mental and emotional challenges. 

What is the difference between a Type C Community like The Osborn and a Type A Community?

Type A contracts present an all-inclusive bill with high prices and fixed costs. Type C (Fee-for-Service) contracts have lower initial costs and monthly fees, which cover housing and amenities, and flexible market rates for any assisted living or skilled nursing care needed later.

The main difference is that with Type A, you pay in advance for your healthcare, while with Type C, you pay as you need it.

Assisted Living FAQ

Do I need to put money down for Assisted Living?

No. There is no community fee required for Assisted Living at The Osborn, a unique advantage compared to many other communities.

If I transition from Independent Living, what is the cost difference?

The cost difference varies depending on the apartment and individual care needs. Because The Osborn follows a care-inclusive model, pricing may appear higher at first glance but already includes many services that other communities charge for separately.

Do you provide on-site medical care?

Residents in Assisted Living have access to visiting physicians who make house calls at The Osborn. However, residents are free to continue seeing their own doctors if they prefer. While medical providers visit the community, Assisted Living does not employ a full-time, in-house Doctor.

Does Assisted Living provide any nursing care?

Yes. We have a team of licensed RNs and LPNs that can assist you in managing many chronic conditions.  The nursing team works closely with your physician to ensure the highest quality of care while you are a  resident of Grandview Assisted Living. 

Skilled Nursing FAQ

How do I pay for my stay in the Pascuiti Pavilion?

For short-term rehabilitation, you will need a 3-night qualifying stay in a hospital.  After that, we accept Medicare, Medicare Advantage and most commercial insurance plans with respective copays per the insurance plans. Private pay is available for long term (permanent residency), or if you wish to stay beyond your approved amount by the insurance company. We do not accept Medicaid as a payment source. 

What is included in my cost of care?

What are visiting hours in the Pavilion? 

Visitor entry to the Pavilion is available, 24 hours, 7 days a week. However, out of respect for other residents, we strongly recommend that visitors exit the building by 8 PM nightly. 

The Osborn Foundation FAQ

Is The Osborn Foundation a nonprofit organization?

Yes, The Osborn Foundation is a not-for-profit, tax exempt 501c3 organization. EIN #47-4600665. Any donation to The Osborn Foundation will be personally acknowledged and publicly recognized in The Osborn Foundation Report.

Is my gift tax deductible?

Yes, financial contributions are tax deductible to the full extent of the law. You should consult your tax advisor for information about tax deductibility.

Can I make a memorable or tribute gift?

Yes, any gift to The Osborn Foundation may be designated in memory of a loved one, family member, friend, colleague or caregiver. Gifts can be made online via the donate button below. If you are donating by check, please note on the check who you are honoring.

What gift types are accepted?

The Osborn Foundation accepts gifts of cash, securities, and gifts in kind on a case-by-case basis in accordance with our Gift Acceptance Policy.

How can I increase the impact of my gift?

Many companies offer matching gift programs to employees, board members and retirees. Please check with the company you work for, or have retired from, to inquire about the details of their program.
