Visitor Information

General Visitation Information

Visits for Independent Living, The Pavilion Skilled Nursing Center, Assisted Living and the H.O.P.E. Center no longer have to be scheduled in advance. Please contact the appropriate program where your loved one lives for additional details.

Pavilion Skilled Nursing Center
Visitor Information

We are glad to be able to accommodate visitation with your loved ones.  In order to ensure the utmost safety for you and your loved ones, and in accordance with the NYS Department of Health regulations and Centers for Disease Control recommendations, we ask that you please follow these rules for a safe and successful visitation.

The following DOH guidelines will take effect immediately: 

1.  All visitors must wear a well-fitting mask (i.e., surgical masks, KN95, or N95) at all times while in the facility. If the visitor wishes to wear a cloth mask, it must be worn over the surgical mask, KN95, or N95

2.  All visitors must physically distance from facility personnel and from patients/residents/visitors who are not directly associated with the specific resident(s) being visited.

3.  Visitors must show proof of a negative antigen test (rapid test) result 24-hours prior to visitation OR a negative PCR test result 48-hours prior to visitation.  See further details in #4 and #5 below.

4.  Antigen Test (rapid test):

  • Prior to visitation, the visitor will be asked to show proof of a negative antigen test (rapid test) 24-hours prior to visitation date.
  • If the test result is from a test center, the staff member will verify the information on the test and compare it to the visitor's ID.
  • If the test result will be from a self-administration test (home test), the staff member will ask the visitor to write their name on the card, self-administer the test in the presence of the staff member, and wait outside the building for 15 minutes for the results.  Once the results are completed, the visitor will affirm the results by signing the affirmation form.
  • Please be aware that if the result returns positive, the visitor will not be allowed to re-enter the building and visit.

5.  PCR Test:

  • Prior to visitation, the visitor will be asked to show proof of a negative PCR test 48-hours prior to the time of visitation.  If the test result is from a test center, the staff member will verify the information on the test and compare it to the visitor's ID.

6.  For visitors who visit multiple days, including a visitor who comes every day, proof of negative testing is required at a minimum every third day (i.e. on day 1, day 4, day 7, day 10, etc.).

  • Although not required by the DOH, The Osborn is requesting that anyone who visits daily perform an antigen test for the safety of our residents/patients/staff and visitors.

7.  For those loved ones who are receiving compassionate visits (i.e., hospice care), you are exempt from the above testing guidelines.  However, due to the compromised nature of your loved one's condition, we strongly urge visitor(s) to follow the above testing guidelines.

8.  In order to mitigate the spread of COVID-19 and to better meet these guidelines, our preferred visiting hours are from 7:00am-7:00pm.  We are asking all visitors to please work with us and visit during these hours.

9.  Even if a visitor has had COVID-19 within the last 90 days, they still will need to test.  If the result returns positive, the visitor will not be allowed to visit.

We understand that this new guidance may not allow many of you enough time to be tested in order to visit.  Please bring your own test because we have been supplied a limited number of test kits from the DOH and may not have enough tests on hand to test all visitors.  As always, I want to thank you for your understanding and cooperation, as we get through this together.  Please feel free to reach out to me if you have any questions at (914) 925-8215.  Thank you for your understanding and cooperation.

Osborn Assisted Living and H.O.P.E. Center
Visitor Information

New York State Department of Health visitation requirements include:

  • Visitors will have to pass the screening questionnaire and a temperature check. A temperature greater than 100.0⁰ F or the presence of symptoms of COVID-19 infection (cough, shortness of breath, sore throat, chills or new onset of loss of taste or smell) require cancellation of the visit.
  • Residents with COVID-19, or who are suspected of having COVID-19 cannot be visited in the visitation program.
  • All visitors must wear a face mask, covering the nose and mouth at ALL TIMES while on the premises of The Osborn. If you do not have a mask one will be provided to you. We have all proper personal protective equipment available upon request.
  • All visitors must maintain social distancing of at least six feet apart from one another. Hand hygiene is a must, please use the hand sanitizers where available.
  • A maximum of TWO VISITORS per resident are allowed during a single visit time; children count as a visitor.
  • Visitors arriving early for a scheduled visit must wait in their vehicle or in the designated waiting location, maintain 6 feet social distance from others.
  • For the safety of our residents and others, hugging, hand holding, kisses, or other direct contact is NOT permitted.
  • Visitors who arrive late may not be able to visit for the full visitation time if doing so will interfere with another scheduled visit. To the extent possible, the facility tries to accommodate visitation.
  • Any visitors under the age of 16 must be accompanied by an adult 18 years of age or older.
  • Visitors are limited in their movement in the facility. This includes limiting visitors from walking around different hallways or using pantries or restrooms. Visitors will be escorted to and from visit location (use of restrooms are not permitted).
  • If bringing food or flowers to a resident, those items are left at Front Desk on Main Floor and will be delivered to the resident.
  • If you should test positive for COVID-19 or exhibit symptoms of COVID-19 in the next 14-days, you MUST notify the facility.
  • If any visitor fails to adhere to the visitation protocol, visitation will be terminated and the visitor will be asked to leave the premises. Visitor may be prohibited from visiting for the duration of the COVID-19 state declared public health emergency.

Osborn Sterling Park - Independent Living 
Visitor Information          

We are pleased to inform you that COVID-19 visitation restrictions for Sterling Park Independent Living have been relaxed. This decision is based on the fact that 99% of Sterling Park residents are fully vaccinated for COVID-19. However, we ask that you help us to reduce the spread of COVID-19 and flu on our campus, please refrain from visiting you loved one if:

1. You are not feeling well (cold, sore throat, sneezing, cough).

2. You have been exposed to a family member or had close contact with someone who has recently rested positive for COVID-19.

Your cooperation and patience during this pandemic has contributed enormously to keeping our community safe and healthy. Please be advised that The Osborn can, at its discretion update or change  its visitation policy for Sterling Park as it sees for the benefit of its residents.
