Visitor Information

General Visitation Information

Visits for Independent Living and The Pavilion Skilled Nursing Center no longer have to be scheduled in advance. The Osborn’s Assisted Living Program still requires that you schedule your visit in advance on our Scheduling Portal.  The Scheduling Portal has specific explanation on where the visits will occur. Please contact the appropriate program where your loved one lives for additional details.

Pavilion Skilled Nursing Center
Visitor Information

We are glad to be able to accommodate visitation with your loved ones.  In order to ensure the utmost safety for you and your loved ones, and in accordance with the NYS Department of Health regulations and Centers for Disease Control recommendations, we ask that you please follow these rules for a safe and successful visitation.

The following DOH guidelines will take effect immediately:

1.  All visitors must wear a well-fitting mask (i.e., surgical masks, KN95, or N95) at all times while in the facility. If the visitor wishes to wear a cloth mask, it must be worn over the surgical mask, KN95, or N95

2.  All visitors must physically distance from facility personnel and from patients/residents/visitors who are not directly associated with the specific resident(s) being visited.

3.  Visitors must show proof of a negative antigen test (rapid test) result 24-hours prior to visitation OR a negative PCR test result 48-hours prior to visitation.  See further details in #4 and #5 below.

4.  Antigen Test (rapid test):

  • Prior to visitation, the visitor will be asked to show proof of a negative antigen test (rapid test) 24-hours prior to visitation date.
  • If the test result is from a test center, the staff member will verify the information on the test and compare it to the visitor's ID.
  • If the test result will be from a self-administration test (home test), the staff member will ask the visitor to write their name on the card, self-administer the test in the presence of the staff member, and wait outside the building for 15 minutes for the results.  Once the results are completed, the visitor will affirm the results by signing the affirmation form.
  • Please be aware that if the result returns positive, the visitor will not be allowed to re-enter the building and visit.

5.  PCR Test:

  • Prior to visitation, the visitor will be asked to show proof of a negative PCR test 48-hours prior to the time of visitation.  If the test result is from a test center, the staff member will verify the information on the test and compare it to the visitor's ID.

6.  For visitors who visit multiple days, including a visitor who comes every day, proof of negative testing is required at a minimum every third day (i.e. on day 1, day 4, day 7, day 10, etc.).

  • Although not required by the DOH, The Osborn is requesting that anyone who visits daily perform an antigen test for the safety of our residents/patients/staff and visitors.

7.  For those loved ones who are receiving compassionate visits (i.e., hospice care), you are exempt from the above testing guidelines.  However, due to the compromised nature of your loved one's condition, we strongly urge visitor(s) to follow the above testing guidelines.

8.  In order to mitigate the spread of COVID-19 and to better meet these guidelines, our preferred visiting hours are from 7:00am-7:00pm.  We are asking all visitors to please work with us and visit during these hours.

9.  Even if a visitor has had COVID-19 within the last 90 days, they still will need to test.  If the result returns positive, the visitor will not be allowed to visit.

We understand that this new guidance may not allow many of you enough time to be tested in order to visit.  Please bring your own test because we have been supplied a limited number of test kits from the DOH and may not have enough tests on hand to test all visitors.  As always, I want to thank you for your understanding and cooperation, as we get through this together.  Please feel free to reach out to me if you have any questions at (914) 925-8215.  Thank you for your understanding and cooperation.

Please help us ensure a safe, pleasant visit with your loved one by adhering to all of these safety and infection control requirements. If you have any questions or need assistance or accommodations, please don’t hesitate to ask a staff member.

  • Visitors will have to pass the screening questionnaire and a temperature check. A temperature greater than 100.0⁰ F or the presence of symptoms of COVID-19 infection (cough, shortness of breath, sore throat, chills or new onset of loss of taste or smell) require cancellation of the visit.
  • Residents with COVID-19, or who are suspected of having COVID-19 cannot be visited in the visitation program.
  • All visitors must wear a face mask, covering the nose and mouth at ALL TIMES while on the premises of The Osborn. If you do not have a mask one will be provided to you. We have all proper personal protective equipment available upon request.

  • If you should test positive for COVID-19 or exhibit symptoms of COVID-19 in the next 14-days, you MUST notify the facility.
  • If any visitor fails to adhere to the visitation protocol, visitation will be terminated and the visitor will be asked to leave the premises. Visitor may be prohibited from visiting for the duration of the COVID-19 state declared public health emergency.

Please help us ensure a safe, pleasant visit with your loved one by adhering to all of these safety and infection control requirements. If you have any questions or need assistance or accommodations, please don’t hesitate to ask a staff member.

Osborn Sterling Park - Independent Living
Visitor Information

We are pleased to inform you that indoor family visitation is now possible in Sterling Park Resident Apartments and Garden Homes, following the safety protocols outlined below. This decision is based on the fact that 99% of Sterling Park residents are fully vaccinated for COVID-19.

All visitors will be required to present proof of either: (1) a negative COVID-19 PCR test collected within 7-days of the visit, or (2) vaccination completed more than two weeks before the visit. Your cooperation and patience during this pandemic has contributed enormously to keeping our community safe and healthy.

 Indoor Visitation Requirements

  • Reservations for indoor visits are not required.
  • Visits can be made daily from 9am to 8pm. Please see the instructions below about check-in procedures upon arrival.
  • Visitation is limited to FOUR GUESTS per Apartment/Garden Home.
  • Sterling Park family visitors who are fully vaccinated will be permitted to stay overnight in apartments and garden homes. Residents must notify the Director of Independent Living if they plan to host overnight visitors. Visitors must be screened at the WellSpring Entrance at the start of their stay. These stays should be kept to a maximum of 3 nights and should be reserved for close family members only, not to exceed more than 2 guests per overnight visit. The guests will not be able to use any of the Common Areas or Dining Rooms at this point. Residents are responsible for ensuring their guests are following these guidelines.

 Upon Arrival

  • All visitors must report to the WellSpring Entrance to complete and pass The Osborn’s screening process before proceeding to the resident’s Apartment or Garden Home.
  • At the WellSpring Entrance, you must present proof of vaccination or a negative COVID-19 test to be allowed entry.
  • All visitors from out of state must follow NYS Travel Guidelines (see NYS COVID-19 Travel Advisory - https://coronavirus.health.ny.gov/covid-19-travel-advisory).
  • Visitors who pass the screening process will be issued an identification sticker that must be worn at all times during the visit.
  • Visitors must use the hand sanitizer located at the check-in area.
  • Visitors who pass the screening process need to exit the building and drive directly to the closest entrance of the Apartment or Garden Home they are visiting.
  • If visiting a Garden Home, entering any other Osborn building (other than for check-in at the WellSpring Entrance) is prohibited.

 During Visit

  • All visitors, regardless of vaccination status, must still check in at the WellSpring Café to be screened. Visitors are still required to wear their masks indoors until they are in the private independent residence.
  • Social distancing of no less than 6-feet must be observed at all times unless all parties are vaccinated.
  • Residents and guests must not ride the elevator if another resident or staff person is inside that elevator.
  • Under no circumstances are indoor visitors permitted in any common space, café, dining area, community restrooms, mailroom, etc.
  • Eating food together is not advised; communal sharing of food is prohibited.

The resident who is receiving guests accepts responsibility to minimize contact between their guests and other residents. Residents and visitors should make every effort to continue to meet outdoors, weather permitting, as outdoors is still the safest place to meet.

Your continued cooperation and understanding are greatly appreciated. These guidelines are intended to provide opportunities for residents and families to be together while minimizing health and safety risks for our whole community. The Osborn reserves the right to alter these guidelines or restrict or rescind visitation at any time. We look forward to welcoming your visitors.

For specific questions about visitation, please reach out to a member of the Sterling Park staff:
